To participate in our programs, hard copies of the following must be received by June 30, 2023:
- Pop Warner Contract,
- Pop Warner Medical Form (signed by pediatrician after January 1, 2023)
- BuxMont Emergency Treatment Form
- Copy of player birth certificate (even if provided previously)
- Recent 2x3 player photo (paper copy and uploaded to TeamSnap)
- Copy of 2023 final/year end report card (Tackle and Cheer ONLY)
